EXPERT WITNESS PRELIMINARY REPORT: CRACKS WERE OVERLOOKED According to the preliminary expert witness report prepared by a team including occupational safety experts, environmental, geological, construction, and agricultural engineers who conducted examinations at the mine site, 5 individuals, including the process oxide manager who "failed to take timely precautions for the cracks in the heap," were found to be primarily at fault. The report stated that the acting operations director, as an authorized person, exhibited careless behavior regarding the cracks that occurred in the heap in the morning, which could lead to adverse effects at the construction site. It was noted that he was found to be primarily at fault for the accident. The report also described how the process oxide manager, despite being aware that the occurring cracks would pose a significant risk, ignored them and exhibited dangerous behavior. It was concluded in the report that this individual was primarily at fault in the incident. Additionally, it was emphasized that the oxide operation chief engineer, despite being repeatedly warned by the occupational safety expert about the cracks in the heap, provided a solution and accelerated the movement in the heap, disregarding the cracks occurring at regular intervals. It was determined that he disregarded this risky situation, endangered the safety of the workers by ignoring the evacuation of the area, and was primarily at fault in the accident.
